REQUIREMENTS FOR: ADULT PASSPORT (Age 16 and OLDER)

An applicant who is who is 16 years or 17 years old must have 1 parent to attend the initial sealing process at a post office or clerks office. Applicants 16 years and up will receive a passport good for 10 years.

 

.         1. ONE (1) STATE APPLICATION FORM DS-11 Must be typed on-line then printed. **NO HAND FILLED APPLICATIONS**. Don’t leave blank answers. Do not sign the DS-11 application until requested to do so by the agent at the Post Office or County Clerk’s Office.

      2. PROOF OF CITIZENSHIP A) Certified U.S. Birth Certificate (Long Form) issued by the state or county in which you were born. Birth Certificate must have a raised embossed seal, file number, and registrars signature. B) Expired Passport issued over 15 years ago, you can use that in place of the birth certificate.   C) Original Naturalization Certificate you will get it back with the completed passport). (Married female applicants must provide original marriage license)

    

      3. ONE (1) PASSPORT PHOTO – In COLOR, size 2 X 2, must have a “white” background. Photos must be recent and taken within the past six months. *No hats or scarves unless for religious purposes.*

      

      4. ONE (1) LETTER OF AUTHORIZATION: (Select Reference Menu) Confirm the first 2 middle boxes that state “I authorize…” are checked. Below please write the traveler’s name, the phone number, TODAYS date then  sign the bottom line. 

 

5.  PROOF OF IDENTITY: The client must provide proof of Identification in the form valid driver’s license from any state with photo.   

6.  PROOF OF DEPARTURE: Original Tickets, E-Tickets, Cruise Confirmation or Hotel Reservation must be presented to the acceptance agent if passport is required within two (2) weeks. You will need to bring a copy so the acceptance agent can place it inside the sealed envelope, and return to you the originals. 

7.  STATE FEES – See Postal Visit Check List on the Reference Menu 

REQUIREMENTS FOR: MINOR PASSPORT (Age 15 and under)

 A personal appearance by both parents and the minor is required to have the forms reviewed and sealed before submission. Childrens passports are valid for 5 years and not renewable. You must re-apply as a new first time passport applicant. (Minors 16 and 17 years old will apply for an adult passport with one parent present)

Do not sign the DS-11 application until requested to do so by the agent at the Post Office or County Clerks Office. (The parents’ signatures will be witnessed in the presence of the government passport acceptance agent).

        2.  PROOF OF CITIZENSHIP – ORIGINAL CERTIFIED U.S. BIRTH CERTIFICATE: Long form required. The Birth Certificate must contain the following: Parents names, State or County Seal, Hospital name, Filing Date and Registrars Signature. Submit childs previous passport (if available). The old expired passport will be returned.

 

      3. ONE (1) PASSPORT PHOTO In COLOR, size 2X2, must have a “white” background. Photos must be recent and taken within the past six months. *No hats or scarves unless for religious purposes.*

     

      4.  ONE (1) LETTER OF AUTHORIZATION: Signed by a parent or legal guardian. Confirm the first 2 middle boxes that state “I authorize…” are checked. On the bottom please write the Childs name, the Parents phone number, TODAYS date and sign the bottom line as your signature appears on your i.d..

 

      5.  PROOF OF IDENTITY: The parents must provide “Parental Identification” (for minors under age 16). Proof of Parental Identification in the form of valid driver’s license from any state with photo. (If last name on license differs from child, law requires presentation of legal documents for proof of parentage).

 

        6.  PROOF OF DEPARTURE: Original Tickets, E-Tickets, Cruise Confirmation or Hotel Reservation must be presented to the acceptance agent if passport is required within two (2) weeks.  

 

7. STATEMENT OF CONSENT FORM DS-3053: To be completed ONLY If one of the parents are not present. The non-appearing parent must sign in original and have this form signed in the presence of a notary public. This form must be notarized. The non-appearing parent must provide a copy of the front and back of the valid identification.

 

8.  STATE FEES – Have check or money order in the amount of $178.32 to “U.S. Department of State”. Also have payment for the agent in the amount of $35.00. (We can write checks if requested)

 

SEALED ENVELOPE PROCESS TO EXPEDITE: Bring the Sealed Envelope to us.
