Postal Visit Checklist

HAND CARRY PROCEDURES FOR POST OFFICE
Tell agent this is a hand carry / third party expedite process
(Upon completion pay the clerk $35.00 and they will hand you the sealed envelope)
Items to be sealed in the envelope by a clerk:
   1. DS-11 application (typed with 2-D bar code in top left corner of application)
   2. Photo – 2” x 2” (no eyeglasses or head covering unless for religious purposes)
   3. Letter of Authority from 3rd party company (Cliffs Passport Service)
   4. Birth Certificate (+ a copy)
   5. Old passport (+ a copy if applicable)
   6. Proof of Travel (flight itinerary with foreign destination or Company Letter)
   7. Valid i.d. (driver’s license, passport or State issued i.d. and a copy front and back)
   8. *Payment (check or money order in the amount $209.53 to “U.S. Department of State”)
        *(Put applicants name and birthdate on the memo line)
Passport Book only
        *(Adult passport expedite fee is $209.53) (State fee $190.00+$19.53 for 1-2 day return shipping)
        * (Minor passport expedite fee is $179.53) (State fee $160.00 +$19.53 for 1-2 day return shipping)
Passport Book and Passport Card
        *(Adult passport expedite fee is $239.53) (State fee $220.00+$19.53 for 1-2 day return shipping)
        *(Minor passport expedite fee is $194.53) (State fee $175.00 +$19.53 for 1-2 day return shipping)
Passport Card only
